You have the right to request access to your health care records.
To request a copy of a child’s health care records from Dot & Dash Speech Therapy, please contact:
Please include the child’s full name, date of birth, and the type of records you are requesting. For your privacy, please do not send medical or clinical information through the website contact links when making your initial request.
Dot & Dash will process requests for access to health care records in accordance with applicable federal and Texas law. Additional information or verification of your legal authority to request the records may be required.
Texas Notice
Texas law provides certain rights regarding privacy of and access to health care records (Texas Health & Safety Code, Chapter 181, Medical Records Privacy). Contact the Texas Office of the Attorney General, Consumer Protection Division to file a consumer complaint alleging a violation of your privacy rights regarding protected health information.
Report suspected violations of statutes or rules enforced by the Texas Department of Licensing and Regulation by contacting the Department or calling (800) 803-9202.
Fees for Records
Dot & Dash may charge a reasonable fee for certain copies or record-related requests when permitted by applicable law. You will be informed of any applicable fee before it is charged.
